My Health Online allows you to book GP appointments and request repeat prescriptions online using your mobile, tablet or computer. It means that you don't have to wait on the phone to your GP practice - you can make requests online at a time that suits you.

Returning the Registration Form and Identification Documents:
Return the completed form to Reception along with TWO identification documents such as a passport or driving licence and something with your address on like a bank statement (but not a gas or electric bill).
One we have received your form and verified your identification documents the practice will then register you for a MHOL account.
You will receive an email so you can complete your registration online. Please check your 'junk' mail box if it looks as if you haven't received the email as sometimes they will appear here instead of your main inbox.
Vouching with Confirmation
If you are unable to provide a copy of your identification documents the practice may be able to utilise 'Vouching with Confirmation'. This involves an authorised member of staff asking questions relating to information held in your medical record. Please note this will give you time limited access for an initial six months by which time identity documentation should be provided to the practice or access may be revoked.
